Gynecomastia is the female-type breast appearance in male patients. The reason for this appearance is that the breast tissue grows more than normal. (Female-type breast appearance due to excess regional adipose tissue is called pseudogynecomastia-false gynecomastia or lipomastia, not gynecomastia.) Gynecomastia, the most common breast problem in men, is seen bilaterally in 75% of patients. Gynecomastia can only be treated with surgery.
*Normally, gynecomastia is seen in babyhood, adolescence period and old age (physiologically). However, some drugs and hormonal diseases are among the pathological causes of breast enlargement in men. Lavender can be given as an example of environmental factors that can cause gynecomastia.
* It is very important to understand whether the growth is normal (physiological or due to weight gain) or pathological (due to a known or unknown disease or drug-substance use) in a patient who comes with the complaint of male breast enlargement. Accordingly, some tests are requested and the cause is investigated. Gynecomastia should be performed after the cause of the growth is understood, if no pathology requiring other treatment is detected. If an underlying drug use or disease has caused male breast enlargement, this condition should be treated first.
*Gynecomastia can be performed if breast enlargement in men is present for more than 1 year or if no pathology is detected in the interrogation and examinations. In the case of enlarged breasts in men, two surgical methods can be applied for gynecomastia: liposuction or cutting and suturing the incision.
Two parameters are important in method and technique selection;
1- The severity of breast size (Size, sagging, skin propoerties, etc.)
(note: the Simon classification is used to assess the severity of breast enlargement:
Type – 1: Minimal breast size without excess skin
Type – 2 : a) Moderate growth without excess skin
b) Minimal skin excess and moderate breast enlargement
Type – 3: Excess skin and advanced breast size.)
2- The nature of the tissue that causes breast size (does it have more adipose tissue or breast tissue?)
(note: breast tissue quality is evaluated by Webster classification:
Type – 1 : Glandular (breast tissue)
Type – 2 : Mixed (Glandular + Adipose)
Type – 3: Adipose tissue
*For example, if there is a mild breast enlargement complaint in a patient who does not have any other disease or a history of drug use, if the tissue causing the growth is reported as "adipose tissue" in the USG examination; means that our patient has “pseudogynecomastia”. The surgery can be successfully performed with liposuction, which is a method without incisions and stitches. Liposuction method is performed with devices with different technology that break down fat with heat. They cause reduction in fat tissue with different energies such as ultrasonic, laser beam and infrasonic. There are different advantages and disadvantages of fat removal procedures performed with these devices instead of conventional liposuction.
If the patient who comes with a gynecomastia request has saggy and very large breasts, the excess skin is definitely removed during the patient's gynecomastia. For this reason, a surgical procedure in the form of “cutting-excessive removal and suturing” should be performed with scars in different lengths and localizations. As a result, in male breast growth problem; Breast tissue, adipose tissue, or both tissues in combination may be responsible for growth. After detection of this, liposuction alone, cutting-removal-stitching techniques or combined techniques can be applied in the gynecomastia operation.
*According to the method to be used, the operation takes 1-2 hours. After gynecomastia, patients can leave the hospital the same day or stay for a day. The post-procedure period is not very painful. Pain is controlled with painkillers to be taken orally for a few days regularly. Patients use a corset or bandage for 3-4 weeks and can take a bath on the 2nd day if no drain is placed. If there is a drain, they can take a bath the day after the drain is removed. (Drains are often taken on the 1st-2nd days.) If your doctor prefers waterproof dressing, you can take a bath whenever you want after discharge.
*Patients can return to their daily routines (walking, light work, etc.) within 1-2 days, but heavy work and exercises should not be done for 4-6 weeks.
* If only liposuction is performed, there are no scars because it is worked through millimetric holes in the skin, but if there are incisions and stitches, these suture scars, which are red in the first months, become obscure within 1 year. However, they are not completely lost. In cosmetic surgeries, scars remain on wounds that are quite acceptable, sometimes unnoticeable, but hardly noticeable even with a careful look. On the other hand, there is no technology, medicine or cream that ensures the complete disappearance of surgical scars. In all cases where the skin is cut, scarring is inevitable. However, in some localizations of our body (for example, palms, eyelids, etc.) and in some genetically lucky people, there may be scars that can be perceived very, very hard, and that some inexperienced eyes cannot notice.
* In summary, if you have a gynecomastia problem, if you have an obstacle to surgery in terms of health, if you do not complain about the appearance of your breasts, you do not need to have surgery.
